ACKNOWLEDGMENT AND AGREEMENT
By signing your name below, you acknowledge that you have read, understand, and agree to the following terms, as well as the LeaseWiz, LLC Terms of Service.
1. Authorization to Verify Information
I hereby authorize LeaseWiz, LLC, its employees, agents, and any third-party partners to take all actions necessary to verify the information provided in this application. I understand that such actions may include, but are not limited to:
I agree to hold LeaseWiz, LLC, its employees, agents, and authorized third parties harmless from any liability related to the accurate obtaining, reporting, or verification of this information.
2. Use of Information for Collections
I authorize LeaseWiz, LLC and its agents to use the information provided on this application for purposes of collection activities should such actions become necessary.
3. Accuracy of Information
I certify that all information provided by me in this application is true, correct, and complete. I understand that any misrepresentation, omission, or falsification is grounds for LeaseWiz, LLC to deny this application and/or terminate any agreement, lease, or transaction associated with it.
4. Vehicle Delivery Authorization & Assumption of Risk
I hereby authorize LeaseWiz, LLC, including its directors, officers, employees, representatives, vendors, agents, and affiliated delivery personnel, to pick up and/or deliver the agreed-upon vehicle to and from the dealership and to and from the delivery location designated by me.
I acknowledge and assume all risks associated with transporting or delivering the vehicle to the destination I have selected.
5. Release & Waiver of Liability
In consideration of the services provided, including any delivery fees waived by LeaseWiz, LLC, I agree for myself and on behalf of my executors, administrators, heirs, successors, and assigns to the following:
